Its mouth is in the cities of Porto (right bank) and Vila Nova de Gaia (left bank). In Portugal, its course is 213 km long, advancing from east to west, crossing the cities of Miranda do Douro, Vila Nova de Foz Côa, São João da Pesqueira, Lamego, Peso da Régua, Pinhão and Gondomar.

In the Douro Wine Region - World Heritage Site - the Douro River crosses the cities of Vila Nova de Foz Côa (by road, 198 km from Porto), São João da Pesqueira (160 km from Porto), Lamego (130 km) and Peso da Régua (98 km).
